Who’s Eligible: Children from birth to age 21 years

Services: Physical exams, school physicals and health screenings are available. An exam is comprehensive and requires the following components:
* Health and developmental history
* Unclothed physical exam
* Vision screening
* Hearing screening
* Laboratory tests
* Immunizations
* Health education/anticipatory guidance

Cost: For uninsured - Sliding Fee Based on Income or TennCare or Private Insurance
